In 1988 the American College of Obstetricians and Gynecologists (ACOG) suggested that the four-chamber view should be obtained during second and third trimester ultrasound examinations of the fetus. In 1993, an ACOG Technical Bulletin was updated which reaffirmed the importance of the four-chamber view and introduced the concept of the outflow tract examination. In September, 1998, the American Institute of Ultrasound in Medicine introduced a new educational bulletin in which guidelines for examination of the four-chamber view and the outflow tracts were provided.
Recently,the American College of Obstetricians and Gynecologists, American Institute of Ultrasound in Medicine, American College of Radioloogy, The Royal College of Obstetricians and Gynaecologists, The Society of Obstetricians and Gynaecologists of Canada, and the Australian Society of Ultrasound in Medicine have recommended physicians incorporate the outflow tracts with the four-chamber view in the routine examination of the heart. This presents new challenges since this is one of the most complex components of the fetal cardiovascular examination.
While the above guidelines are helpful, there is little information on how to ACQUIRE the images of the heart given various fetal positions. In addition, there is little discussion regarding the presentation of different types of pathology that may be encountered.
